            Home Visit Program offers “integrated, in-home
            care by Community Paramedics and is available
             throughout the County of Simcoe at no cost to
            the patient. 129 Visits were made to 35 Shelter
              patients in Midland (22). Paramedics receive
             additional training in key treatment areas. They
             maintain a direct connection with the patient’s
            Primary Care Provider and can minimize the need
                 for 9-1-1 calls and hospital visits (21).
